Senior Backend Engineer

We are a physician-led healthcare organization building a modern automation layer on top of our EMR (Nextech Practice Plus). We maintain our own SQL database that mirrors patient, appointment, and workflow data, and we are now implementing event-driven automations to streamline operations across tools like Asana, Google Workspace, and communications platforms. We are looking for a Senior Backend Engineer who can design and build reliable, secure integrations that turn structured data into real operational workflows. This is not a front-end role and not a generic CRUD job. You will be designing systems that watch data, apply rules, trigger actions, and never silently fail. What You’ll Do • Design and build backend services that: • Monitor SQL databases for defined events (appointments booked, upcoming visits, missing documentation, etc.) • Apply business rules (“if X then Y”) • Trigger actions via external APIs (Asana, email/SMS, internal services) • Implement idempotent, fault-tolerant automation (no duplicate tasks, safe retries) • Integrate with third-party APIs using REST + OAuth • Build logging, auditing, and alerting so automations are observable and trustworthy • Work closely with clinical leadership to translate real-world workflows into automation logic • Ensure secure handling of PHI and follow HIPAA best practices (data minimization, access control) Required Qualifications • 5+ years of backend engineering experience • Strong SQL skills (Postgres, MySQL, or SQL Server) • Strong experience with Python (FastAPI) or Node.js (TypeScript) • Proven experience building integrations with external APIs • Experience with: • Background jobs / schedulers • Queues and async processing • Retry logic and failure handling • Solid understanding of system reliability concepts: • Idempotency • Event consistency • Monitoring and logging • Comfortable working independently and owning systems end-to-end Strongly Preferred • Experience integrating with: • EMRs, healthcare systems, or regulated data • Asana, Jira, or workflow/project management APIs • Familiarity with HIPAA considerations (you don’t need to be a lawyer, but you must understand the mindset) • Experience designing “automation engines” or workflow systems • Cloud deployment experience (AWS, GCP, or Azure)
